My boyfriend has been in severe pain for the past 3 days and found out yesterday from a dentist that it's an absessed tooth. He began taking Clindamycin HCL 150 mg about 19 hours ago, and has been on Hydrocodone/Ibup 7.2/200 for pain. The pain seems to have only gotten worse. He discovered yesterday morning that if he puts cold water on the tooth, it temporarily relieves the pain, so he's been continually drinking cold water for 24 hours now. He's in such severe pain that i've called the dentist back a couple times asking if there's ANYTHING else that can be done to alleviate the pain, and they tell me he just needs to wait it out until the antibiotics begin working. A few questions:
1. How long after beginning to take the antibiotics are they supposed to begin working, thus alleviating some of the pain?
2. Is it possible that the continuous cold water is aggrevating the problem?
3. Since he's drinking so much water and in turn urinating a lot, is it possible he's flushing the medicine out of his system?
4. Is there anything else at all that can be done to help alleviate the pain?

Under most circumstances, the best treatment is to go ahead and access the nerve (pretty much start the root canal) and let the abscess drain and taking antibiotic concurrently (simultaneously) to help fight the infection. Once that has healed, then the RCT can be completed.

Did the dentist give him any reasons on why he wouldn't start the root canal to relieve him of the pain?

The dentist scheduled him for a root canal a week from tomorrow because he wanted the swelling and infection to decrease before then. I've talked to the dentist again since posting this and he stated again that the only thing that can be done for the pain is to wait on the antibiotics to begin working. I've been told that the pain meds he's on, Hydrocodone, are some of the strongest so not much more can be done there (is that true? they don't seem to be doing any good at all). After reading other postings, I'm somewhat more reassured now that the antibiotics will begin working eventually....I've heard that it can take a few days before results are noticeable....we were afraid initially that they weren't effective. He's just in sooooo much pain and after being on medication for 24 hours now, it doesn't seem to be getting any better, and from his perspective, it's getting worse. Do you know of anything else that can temporarily help with the pain until the antibiotics kick in.....and so he can get some sleep? He's even suggested a numbing shot like you get when getting a cavity filled.....anything that will give him a respite from the unbearable pain. Thanks for your quick response.

If the pain does not subside within the next 48-72 hours, you may want to consider finding a dentist who is willing to perform the root canal immediately. In my opinion, the primary object is to relieve the patient from pain. There is an existing bacterial infection and although antibiotic helps, it is much more effective to address the source of the problem and open up the tooth to remove the infection, irrigate the canals, and allow for draining and pressure relief.

If the hydrocodone is not working, then my recommendation is to take some over-the-counter Moltrin. 800mg as needed (make sure to read the label and not exceed recommended dosages). Some people are unable to metabolize hydrocodone, thus making it rather useless for pain management. Your boyfriend may fall under that particular category.

wow her boyfriend sounds just like me. i went to the dentist on friday and am scheduled for a root canal on this coming friday. the pain was bad but tolerable on friday, on saturday it was so bad i was in tears and trying just about anything to relieve the pain. my moms boyfriend even offered to pull the tooth out with some string. believe me i was very tempted to let him do it. i also found that holding cold water over that tooth and then swallowing it helped so much. it was my only relief for the whole day. sunday came and the pain was the same. i found myself running to the bathroom every 15 minutes from all of the water i was digesting. i hadnt gotten any sleep saturday night and had to call off of work for monday(today). the dentist told me the same thing, just wait for the medicine to kick in. honestly when he said that i wanted to kick him but today i feel 50% better. so if you know the pain i was in you will know that the 50% is like no pain at all. so my suggestion for all of you going through the same thing is just hang in there. relief is on the way!

I now have an absessed tooth.The pain is severe and I have been using an ice pack with and without towel to try to relieve the pain as well as 500 mg Amoxocillan 2x's a day for the infection and oxycodone for the pain but I have found that Heat works better for me for some reason,I thought it strange but the cold seemed to stop working after a while so I tried heat and it seemed to work better,hope this helps someoen I know the pain is literally unbearable!!

Medications alone are generally not as effective for dental pain or infection as immediate intervention.

I'm not sure if this topic is too old to get a reply, but I have a question along the same lines, but with a catch. I have the same exact problem, but I also have a problem with antibiotics. I can't remember what it is called, but some antibiotics don't work for me. When I got a uinary tract infection it took changing antibiotics three times to get one that actually worked. I also have Lupus, and neutrapenia (found on a test a couple weeks ago). I'm taking Trimox 500mg 3 times a day. I've been taking them for 15 hours and as they stated the pain is so much worse and appears to have spread from my front tooth all the way back behind my ear to where it feels like I have an earache. I just worry about the infection spreading. My aunt had an ear infection that wasn't promptly taken care of and died because the infection spread to her brain.

So in a nutshell, my questions are --

Is Trimox affective for an absessed tooth?
Should I be extra concerned with the neutrapenia?
Should I be concerned with my problem of antibiotics not working affectively? (I honestly don't know which ones don't work, but they did a test on an infection on my leg once and the results came back that I am resistant to some antibiotics. I can't remember the name of the test.)
Should I be concerned with the obvious worsening of the symptoms? (I couldn't see the redness and inflamation last night, and can clearly see it now, and see it goes across several teeth area of my gums, which also bleed like crazy if I touch (touch, not badger) them.)

Thank you SO much for your time. I promise if there is nothing for me to be concerned about I will become patient. I just want to stop worrying.

I am scheduled to see the dentist for the first time on Wednesday (today is Saturday and I called yesturday). I took the inititive to get on the antibiotic via my Dr, knowing that it should give some relief after awhile and that I could never have waited until Wednesday to START working on the infection. Neither my Dr or Dentist are available for questioning until Monday, that's why I came to you.

What a relief to read these reports - I'm now into my second day of an abscess and lurching towards another sleepless night while I down huge amounts of Amoxycillin and painkillers. Cold water is the only relief - and lying down makes things worse. The tooth in question now seems to have become a bit loose - so not sure what that means???!! Am back to the dentist first thing in the morning - but the pain is quite incredible!!! Any other remedies for relieving this much appreciated. xx

As previously stated, antibiotics are an adjunct to definitive dental treatment -- alone, they will not usually manage a dental infection.

If your dentist chooses not to initiate clinical treatment immediately, ask for a referral to the appropriate dental specialist (such as an endodontist, oral surgeon or periodontist, depending on the condition).

Dental abscesses most often require aggressive treatment for relief of pain and infection.

I feel for all of you,I have been there,done that and dont want to go back there again.I had oral surgery last week on an absess that burst in my mouth.and an additional 3 pulled on top of it my jaw was popping,ear hurt and head hurt so badly i literally wanted to end it all.I finally got smart and had the surgery (1490.00)luckily my oral surgeon is working a payment schedule with me but i need addition 8000 worth of work on my mouth done.Anyways....I am no doctor but my antibiotics werent working either and neither were the pain meds so(and i am only telling you what i did...)At night I would put a hot compress on my face take the antibiotic and pain med and in addition to all of that i took excedrin Pm and i was able to atleast sleep.It worked so good for me that the absess burst while i was asleep (probly not sleeping night after night helped)Hope this helps someone

Hi,

I have the same problem. A front tooth started to come loose. It was abscessed.
On Monday my dentist pulled the tooth at 1:30. By 4:30 the pain was unbearable. My husband called him and he prescribed a pain reliever. Tylenol with codine. It worked somewhat after a good hour but would wear off abruptly in less than 4 hours. By 6 PM my face was swollen bad. My neck, cheek, lips and nose were bulging like a cave man. The next day I went back to my dentist and he prescribed Clindamycin Hcl 150 mg capsules. He said I had a bacterial infection. Today is Thursday and all the pain and most of the swelling is gone. I think the Cllindamycin took effect within 12 hours. I don't feel too good. I just want to sleep. But I think I am slowly getting better.

This is my same problem. Severe pain only relieved by cold water. Get this. I called my dentist on March 15th for an emergency appointment. The best they could do is April 8th and I could come in to pick up a prescriptions for hyrocodone and amoxycillin. When I came in to pick up the prescriptions, the receptionist told me she had to cancel my appointment. The dentist is refusing to treat me because I missed my last appointment for root canal without calling. I missed this scheduled appointment because of 4 emergency kidney operations during that 4 months. I've been going to this practice for 7 years. The new dentist can't fit me in until April 5th. I begged my old doctor to see me but the receptionist just said sorry, he won't. 3 weeks of hell. In hell there is ice water- (every 5 minutes all night long forever to relieve toothache pain). Thanks for letting me vent. I am a diabetic and I can't eat. Hydrocodone did nothing, but I ran out of those 20 pills long ago. I wish I was dead. I can't sleep or eat and the pain is unbearable. My dentist is sure teaching me not to miss an appointmnet, huh.

Thank you for the link. Yes, time for a fresh start. I went to the new dentist today. He took X-rays. He says both teeth are absessed. He wanted to pull the one and suggested I go to an endodontist (sp?) for the other, but come back to him for the caps after the root canals. I have no idea why I waited 3 weeks to see a dentist when apparently I needed a specialist all along. He said regular dentists don't do root canal. He said if they do offer to perform root canal, don't let them. Does this make sense? Thank you for helping me. he gave me a prescription for Amoxycillin, Tylenol #3 and Ibuprofen and filled the one tooth to avoid food impaction. However, the other one (the absessed one) has a gaping hole from unfinished root canal. Still in pain.

Some general dentists are skilled in RCT, some are not. A GP is held to the standard of care of the spcialist, if he does specialty work.

May I ask the dentist you saw in Philly? We were there last summer & fell in love with the city. Some great dentists besides.

I've been searching for some assistance for my own similar problem, which was aggravated on Thursday, and which has lead to horror for me since.

Last year, March, 2004, I went to the dental clinic (I have no money or insurance) for an infected front tooth, and they did a "pulpotomy" instead of a root canal. That did work fine--it was only supposed to be temporary (they said to wait for a return appointment, but they didn't tell me it would never come, because they have a "waiting list" for patients, and I was likely never going to get onto the regular patient list) but a year later, nothing else had been done. The tooth began to irritate me about a month and a half ago again, but every time I called, they said there was no emergency dentist that day, and I never got in. I had some left over Amoxicillin which I took, and it seemed to help--the pain subsided for a while, but then both sets of lower back teeth (bottom) began to suffer similar pain, and I was getting frantic.

I finally got hold of the emergency clinic, and I got scheduled for Thursday night. The same dentist who had worked on the tooth a year ago was the one who was working on it this time, and I'm sorry, but the man was incompetent. He had to call in another, more senior dentist the year before to do most of the actual work, but this time, he did it, without assistance, and without proper anesthesia. I also suspect he was rougher than he needed to be with the small brush or whatever it was he was using to infiltrate the tooth pulp. I went home afterwards without ANY after care instructions, without ANY thing at all. Ten minutes after I got home, I was in tears and massive pain--so much so that I had to call the regular hospital and see what they could tell me. I was told if the pain was horrific to go to the ER. I tried going to bed (around 9:15) but at 11:30, I was still lying in bed awake, crying and in pain. I drove to the ER, where the best they could do was to give me some pain pills, and send me home. Yesterday morning, I called the clinic again, and told them I wanted to see another dentist, immediately, and no excuses. I ended up arguing with them, because they said the earliest appointment was at 5:30 last evening, and I said, NO WAY. So I went down, and they finally gave me an appointment at 10:30 yesterday morning. So when I went back, the new dentist said she couldn't do anything with it, because the swelling and infection was so bad that she said the anesthesia would not work, and gave me a prescription for Clindamycin and Tylenol with codeine making another appointment for me for this coming Tuesday.

I now possess an entire infected lower front jaw affecting most of the entire mouth, but in particular the 6 front teeth area, with one tooth (the front left) being the main culprit. It's rock hard in texture if you touch my chin area, and seeming to spread out a little at a time. I just took my third Clindamycin, and third Tylenol #3, and all I can do is wait it out with some sort of stoic resolve, even though I feel so bad that if I had a gun, I just might use it.

What I can't understand is the knocklehead who did the pulpotomy on Thursday night.....why the devil didn't he do more than try to clean out an area without help, and why wasn't he aware that there was a tremendous amount of infection present? I made a point to tell the clinic I never wanted to see that person again, and I am seriously thinking of filing a complaint with the Dental board. If that doesn't do any good, I might just decide to sue that particular dentist for the agony and hell I've been going through since.

I am also guilty of ignoring my tooth pain off and on over the last year until the pain could no longer be ignored. I had a constant pain in my lower jaw (along with swelling) and a sharp pain if the particular tooth was touched. I took alot of Advil until my dentist could see me. He started the root canal to relieve the pressure of the absess and put me on penicillin and loritab to clear up the infection so he could finish the root canal and crown. This worked great for about three days. Then the tooth started hurting again and the swelling came back. The dentist put me on a stronger antibiotic to get rid of the infection. It is now five days later and I still have the swelling. It feels like a hard knot. My tooth has sensitivity to it when I bite down. I cannot eat on that side of my mouth. Should I call my dentist for even stronger antibiotics? I'm not scheduled to see him for another two weeks.

I have TMJ. I have a tooth which the nerve is exposed in the back. Its been severely hurting for the last 2 months. About that long I have had a nasty taste in my mouth. My friend told me that its prolly absessed. I went to the denist 6 weeks ago and he told me that I needed it pulled but he didnt have time. Is there anything I can do to hurry up the prosess. Cause by the way it sounds if it is absessed he wont pull it. I have severe pain from the tmj as well. So maybe alot of my pain is that as well. I dont know I had to ask and get an oppinion. as for the cold water. That hurts my mouth so bad. Idk what to do. Just looking for some advice.

I have an absess that has swollen the left side of my face, there isnt really any pain but the swelling is pretty bad. I do not have insurance and cannot afford a trip to the dentist, is there any thing I could do from home to get the swelling to go down?

Have been reading the posts to get some idea if what I'm experiencing is typical. Here's the history:
- had crowns put on my upper right, back 4 teeth at which time my dentist, who I think is very good, said we might be doing root canals on a couple of those same teeth down the road;

- sure enough, about two weeks later, my cheek began swelling from about my tooth line up through to my nose and the bottom of my eye with pain to go with it;

- went back and we decided to do the root canals;

- had them done Friday afternoon. Pain began Saturday morning along with some swelling in my cheek but not quite as much as before.

- finally called my dentist on Sunday morning and started on Clindamycin Sunday afternoon (300 mg 4x a day).

It's now Monday night so I've been on the antibiotic for 6 doses with almost no relief. Is this typical? Is the antibiotic not working? How long should I continue with this antibiotic before trying something else?

Wow, I feel so lucky. I have an abscesed molar that was killing me, the pain went away with constant peroxide/water (half cup of each) rinses over a period of days- sometimes putting the peroxide straight on my toothbrush with the toothpaste.
But then the swelling started. One bad night and one bad day of swelling, started on penecilin, kept up the rinses and now two days later the swelling is almost entirely gone and zero pain. Gotta get this pulled asap.

I had a root canal on a lower back molar in 1994 while overseas. When i visited my dentist in america he said something was wrong with it and i needed to the root tips cut off. i had this done 1996. this past year i noticed a pimple on the front of the gum of this tooth. not knowing what it was and having no pain, i ignored it. before leaving to return overseas i visited my dentist in the states. he told me it was an absess and that i should have it checked and probably would need to have it pulled and have an implant or bridge. upon arriving back overseas i visited a root canal dentist and she told me she would clean the canals and then see what would happen to this fistula (sp?) and that perhaps the tooth could have a microscopic crack that wouldn't show with the x-rays, or that i could just have it pulled and get the implant. i opted for the canal cleansing. from what she could see all the canals were clean, but the fistula didn't go away...it just kinda shrunk and looked like a scar. so now i'm getting a crown put on in a couple of weeks. however, a couple of days ago this temp crown began hurting (though not unbearable like these other testimonies) and the fistula is very tender. the pain was like a dull, cold, shooting up in my ear type...but then i have tmj...my husband told me to lift up on the temp crown and it did help...today i have no pain...but do u think i'm wasting my time and money with this crown? any suggestions as to what is going on?

I have a similar situation. Except my absessed tooth doesn't hurt unless I put some light pressure on it and isn't swoolen very much anymore. I just started a new job a few days ago and I don't have any money nor any inssurance for at least a month to have the tooth removed. After ready all these posts I was wondering if there are any over-the-counter type antibiotics I can get to take down the swelling until I can get my insurance and an appointment?

Also, I was wondering....what type of dentist do I need to get all dental work done (including root canal & general cavaties) that has sleeping gas (laughing gas doesn't have any effect on me). I would like to simplify all the work by using only one dentist to get all work done before this contract job ends and I run out of insuranc, AGAIN (this keeps happening to me and no matter how much I try to take care of my teeth the more work I will need since I never have insurance long enough to get it all done in seperate appointments). Can anyone help me out with this? I would be most appreciative of it...thank you in advance

I know this is an old thread - but I figured since I have been through 4 root canals I could offer a little advise for anyone else reading this:

1) The pain sucks - but it does go away, I promise. There is little worse than the pain involved with an Absessed tooth. Hydrocodine (Vicodin) is about the best pain medicine that any dentist will ever perscribe to you so go ahead and take it - it doesn't make the pain go away but it makes you not care as much and most importantly it makes you sleepy so you can just try and sleep it off until the antibiotics kick in.

2. Antibiotics are good. Clindamycin is rough on the stomach and might make you feel real nautious all the time - but ask yourself this - would you rather have some nausea and be over the severe toothache in about 72 hours or have that toothace get worse and worse every day. Like I said - Antibiotics are your friend.

3. And a couple of tips to alleviate the pain outside of the medicine are (from personal experience):
a) Elevate your head while you sleep. Chances are you are not sleeping well - this should help some.
b) Ice Ice Ice! 20 minutes of icing will usually net about 60 minutes of relative comfort - on the "bad nights" this might be all you can do - ice, sleep for an hour, wake up in pain, ice, sleep for an hour. Rinse. Repeat
c) Buy Sensodyne toothpaste - This is designed for sensitive teeth, it has a pretty good numbing effect. When I am having a terrible toothache I usually wake up to brush the tooth with this stuff at least 5x a night
d) Keep busy. If you can avoid it, don't stay home from work or school or whatever and feel sorry for yourself. It hurts less if you stay busy and have your mind on other things - I promise.


And lastly - your not alone, thousands of others are going through the same thing right now. It sucks - but its temporary. It will get better, you will have the root canal and you will be happy again.

Hope this helps someone out there!

My husband is going through a terrible toothache and infection. The right side of
his face is very swollen. He went to the Dentist on this Thursday and was prescribed 500MG of Penicillin VK 3 times a day for 7 days
and Hydrocodone for pain. He's still in a lot of pain. the Hydrocodone isn't doing anything.I tried giving him a ice pack I don't know if that did any good either. I hate seeing him in pain like this.

I also have an absessed tooth, the pain is not as bad as the last one,back in March but teh swelling is pretty bad. I can't smile. I was up ever 2 huors last night. I am going to try the cold water. I just tried heat, it helped a little.
